THERAPY OF DISTAL TOOTH DISPLACEMENTS USING ALIGNERS IN ADULT PATIENTS WITH STABLE OCCLUSION

A comparative analysis of the effectiveness of molar distalization using individually programmed aligners in adult patients with intact occlusion was conducted. The study included 28 patients diagnosed with dentoalveolar form of Class II, Division 1 malocclusion. The planned amount of distalization was 2 mm, with actual movement performed in 0.25 mm increments per aligner set, replaced every two weeks. The first group was treated with aligners without attachments, and the second—with vertical attachments on molars. The mean percentage of planned movement achieved was 86.9% in the first group and 88.4% in the second; the average deviation from the planned trajectory was 0.46 mm and 0.38 mm, respectively. These results confirm improved accuracy and biomechanical control when attachments are used. The preserved occlusal relationships within the sample allowed for objective assessment of the impact of retentive elements without the influence of occlusal compensation mechanisms. The findings support the incorporation of retentive modifiers in treatment algorithms for distalization in adult patients with stable occlusion.
