OYOQLARDA SPASTIK SINDROMINI DAVOLASHDA DIFFERENSIAL YONDASHUV

Tadqiqotning maqsadi bolalar serebral falaji (BSF) bilan og‘rigan bolalarda oyoqlarning spastik sindromlarini klinik va neyrofiziologik mezonlar asosida jarrohlik yo‘li bilan davolashga differensial yondashuvni asoslashdan iborat. Prospektiv tadqiqotga BSF kasalligining turli shakllari bilan kasallangan 75 nafar bemor kiritilgan bo‘lib, ularga yetakchi patogenetik mexanizm va spastiklik darajasiga qarab selektiv dorsal rizotomiya (SDR), selektiv periferik nevrotomiya yoki botulinoterapiya o‘tkazilgan. Samaradorlikni baholash MAS, GMFCS, MACS klinik va funksional shkalalari, shuningdek, elektromiografiya ma’lumotlari yordamida amalga oshirildi. Olingan natijalar konservativ usullarga nisbatan jarrohlik aralashuvlarini qo‘llashda, ayniqsa aralashuvning aniqligi va xavfsizligini ta’minlaydigan intraoperatsion neyromonitoringdan foydalanganda spastiklikning yanada aniq va barqaror pasayishini ko‘rsatdi. Ushbu tadqiqot kompleks klinik va neyrofiziologik baholash asosida jarrohlik taktikasini individual tanlashning maqsadga muvofiqligini tasdiqlaydi.
