IMPACT OF GLAUCOMA ON THE DEVELOPMENT OF BLINDNESS IN PATIENTS WITH DIABETIC RETINOPATHY

Relevance: Glaucoma and diabetic retinopathy (DR) are among the leading causes of irreversible blindness worldwide. Both conditions independently contribute to severe vision impairment, but their coexistence may accelerate progression to blindness. Understanding the interaction between these diseases is essential for improving patient outcomes.
Purpose: This study aims to assess the impact of glaucoma on the development of blindness in patients with DR, focusing on how elevated intraocular pressure (IOP) and systemic factors contribute to visual impairment. The goal is to identify key risk factors and suggest early intervention strategies.
Materials and Methods: A meta-analysis was conducted using data from observational studies on patients over 40 years old with both glaucoma and DR. Data included ophthalmic examinations, IOP measurements, and systemic health parameters (HbA1c, blood pressure). Literature from PubMed, Medline, and Scopus was analyzed to evaluate disease progression and the role of systemic and ocular factors.
Results: Patients with both glaucoma and DR experienced blindness significantly earlier (mean age 63.85 years) compared to those with only one condition. Hyperglycemia and elevated IOP were identified as critical contributors to visual deterioration. Male patients and those with poor glycemic control showed a higher prevalence of blindness, with open-angle glaucoma being the most common form in diabetic patients.
Conclusion: The coexistence of glaucoma and DR significantly accelerates the progression to blindness. Early detection, rigorous IOP management, and control of systemic conditions such as diabetes and hypertension are essential to prevent vision loss. These findings highlight the need for integrated clinical management strategies and regular screening in at-risk populations.
