ВИРУС ПАПИЛЛОМЫ ЧЕЛОВЕКА И РАК РОТОГЛОТКИ: СОВРЕМЕННЫЕ ВЗГЯДЫ НА КЛИНИКО-ЭПИДЕМИОЛОГИЧЕСКИЕ И ПРОГНОСТИЧЕСКИЕ АСПЕКТЫ

Аннотация

Цель: Оценить влияние комбинированной схемы лечения на маркеры воспаления и метаболические параметры у пациентов IV стадии рака яичников, эндометрия и шейки матки, находящихся в состоянии прекахексии. Материалы и методы: Проведено проспективное Аннотация: Рак ротоглотки в последние десятилетия характеризуется выраженным изменением эпидемиологического профиля, обусловленным ростом доли вирус папилломы человека (ВПЧ)-ассоциированных форм. ВПЧ-позитивный рак ротоглотки в настоящее время рассматривается как самостоятельный клинико-биологический подтип опухолей головы и шеи, отличающийся по факторам риска, морфологическим характеристикам, клиническому течению и прогнозу. В обзоре обобщены современные данные о распространённости ВПЧ-ассоциированного рака ротоглотки, биологии вируса и механизмах ВПЧ-индуцированного канцерогенеза, клинико-морфологических особенностях и диагностических подходах. Особое внимание уделено прогностическому значению ВПЧ-статуса, роли генотипа вируса, вирусной нагрузки и физического статуса ВПЧ в формировании клинических исходов, а также современным концепциям персонализированного лечения и деэскалации терапии. Показано, что интеграция клинических, морфологических и молекулярных характеристик ВПЧ позволяет углубить прогностическую стратификацию больных и оптимизировать лечебную тактику. Подчёркивается необходимость проведения регионально ориентированных исследований для уточнения эпидемиологических и клинико-прогностических особенностей ВПЧ-ассоциированного рака ротоглотки.
