УЛУЧШЕНИЕ ИНТЕНСИВНОЙ ТЕРАПИИ ПАЦИЕНТОВ ПОСЛЕ АОРТОКОРОНАРНОГО ШУНТИРОВАНИЯ (ОБЗОР ЛИТЕРАТУРЫ)

Среди причин смертности взрослого населения ишемическая болезнь сердца (ИБС) занимает одно из ведущих мест. Международные и российские рекомендации по лечению ИБС требуют коллегиального подхода к выбору метода хирургической реваскуляризации как аортокоронарное шунтирвание (АКШ). В целом, АКШ с включенной помпой и без нее - это 2 типа хирургических вмешательств, разница между которыми заключается в использовании системы искусственного кровообращения (ИК) и остановке сердца для проведения операции во время АКШ с включенной помпой и операции на работающем сердце. Хирургические вмешательства и ИК - это медицинские состояния, которые вызывают системную воспалительную реакцию, активируя систему комплемента и запуская выброс цитокинов. IL-6, IL-1b и фактор некроза опухоли-альфа (TNF-a) являются основными медиаторами острой фазы воспалительных реакций. Анализ показывает, что использования улинастатина в раннем послеоперационном периоде ингибирует секвестрацию и активацию нейтрофилов и ослабляет нормальное послеоперационное повышение уровня цитокинов, уменьшая синдром системной воспалительной реакции, проницаемость микрососудов легких и послеоперационный отек легких.
