TUG‘RUQDAN KEYINGI O‘TA KUCHLI QON KETISHDA INFUZION-TRANSFUZION TERAPIYA STRATEGIYASI

Homiladorlikning 37–38 hafta muddatida yo‘ldoshning o‘sib kirishi yuzasidan akusherlik amaliyotlarida, 115 ta tug‘ruqdagi va tug‘ib bo‘lgan ayollarda prospektiv ikki tomondan yopiq randomlashtirilgan nazorat qilinadigan klinik tadqiqot o‘tkazildi, I-guruh – 57 ta, II-guruh – 58 ta. Anesteziya xavfi ASA bo‘yicha II darajali. Anesteziologik yordam sifatida, guruhlarda LII–LIV darajada spinal anesteziya qo‘llangan, 0,5% Longokain Xevi eritmasidan foydalanilgan, ad'yuvant sifatida Kvanadeks 10 mkg ishlatilgan. Preinfuziya sifatida I guruhda Reosorbilaktning 15–20 ml/kg kristalloid eritmasi, II guruhda 10–15 ml/kg balanslashtirilgan polielektrolitli giperosmolyar kristalloid eritmasi kiritilgan. Amaliyot bosqichlari davomida kristalloidlar va qon komponentlari qo‘shimcha kiritilgan. Bunda markaziy gemodinamika o‘rganilgan, KIH, qon elektrolitlari, laktat, glyukoza va gematokrit “BGA-103” Wondfo (Rain Sen Da) gazoanalizatorida o‘rganilgan. Miya natriyuretik peptid progormoni (NT-proBNP), Sistatin S, S100β oqsili Finecare™ FIA Meter Plus FS 113 (Wonfo) immunoflyuorestsentli analizatorda aniqlangan. Yangi tug‘ilgan chaqaloqlar Apgar shkalasi bo‘yicha va S100β oqsili bo‘yicha Finecare™ FIA Meter Plus FS 113 (Wonfo) da baholangan. Buyrakning funktsiyasi qon zardobidagi mochevina, kreatinin, tugunchalarda filьtrlash tezligi (TSistatin S) va mikroalьbumin bo‘yicha Finecare™ FIA Meter Plus FS 113 (Wonfo) da o‘lchanib baholangan. Gemostaz tizimi OCG-102 (Wonfo) optik koagulometrda o‘rganilgan (Point-of-care Testing).
