SIGNIFICANCE OF SERUM MARKERS OF THE PROINFLAMMATORY CYTOKINE CASCADE IN THE DIFFERENTIAL DIAGNOSIS OF PARKINSONIAN SYNDROME

The article presents a comparative analysis of the pathogenetic features of systemic inflammatory reactions in degenerative and vascular forms of parkinsonism. The profile of proinflammatory cytokines was investigated as the central mediator of immune activation in relation to clinical and neurological manifestations of the disease and gastrointestinal triggers. Based on the results of statistical analysis, a qualitative polarity in the architecture of correlation profiles between the studied nosological groups was proven. The expression of proinflammatory cytokines in Parkinson's disease appears to be systemically intertwined with gut-related factors—specifically infectious invasion and localized mucosal immune activity—as well as clinical symptom severity. Conversely, gastrointestinal influences showed zero impact on cytokine concentrations in cases of vascular parkinsonism. Consequently, the inflammatory response in the vascular group can be classified as an autonomous vasogenic process stemming from endothelial damage and persistent brain hypoperfusion. It is concluded that in vascular parkinsonism, systemic cytokine pooling occurs in isolation from enteral triggers and has an autonomous vasogenic nature caused by chronic cerebral ischemia and endothelial dysfunction.
