STEPLERLI GEMORROIDOPEKSIYA VERSUS MILLIGAN–MORGAN GEMORROIDEKTOMIYASI: UZOQ MUDDATLI ANATOMIK VA FUNKTSIONAL NATIJALAR BO‘YICHA SISTEMATIK TAHLI

Gemorroy klinik amaliyotda eng keng tarqalgan anorektal kasalliklardan biri bo‘lib, global tarqalishi 4,4% dan 36% gacha. III–IV darajadagi gemorroyni jarrohlik davolash an’anaviy ravishda Milligan–Morgan gemorroidektomiyasiga (GMM) asoslangan bo‘lib, u aralashuvning “oltin standarti” sifatida tan olingan. 1998-yilda Antonio Longo muqobil usulni taklif qildi — staplerli gemorroidopeksiya (SGP, yoki PPH protsedurasi), bu esa postoperatsion og‘riq sindromini sezilarli darajada kamaytiradi va tiklanishni tezlashtiradi. Ushbu sistematik sharh ikkala usulning uzoq muddatli anatomik va funksional natijalarini tasodifiy nazoratli tadqiqotlar, meta-tahlillar va perspektiv kohort tadqiqotlar ma’lumotlariga asoslanib taqqoslaydi. Ma’lumotlar to‘plami aniq kompromisni ko‘rsatadi: SGP qisqa muddatli davrda afzalliklar beradi — kamroq og‘riq, operatsiya vaqtining qisqarishi va ish qobiliyatining tez tiklanishi, — ammo uzoq muddatli qaytalashlar va qayta aralashuvlar chastotasi sezilarli darajada yuqori, ayniqsa IV darajadagi gemorroyda. GMM rivojlangan kasallik shakllarida uzoq muddatli radikal natijalar bo‘yicha ustunligini saqlaydi. Sharh natijalari gemorroy darajasi va bemor afzalliklarini hisobga olgan holda usulni tanlash uchun differentsiyalangan algoritmni shakllantirish imkonini beradi.
