ВЫСОКОРАЗРЕШАЮЩАЯ АНОРЕКТАЛЬНАЯ МАНОМЕТРИЯ (ВРАМ): ЕЁ РОЛЬ В ПРЕДОПЕРАЦИОННОЙ ОЦЕНКЕ ФУНКЦИОНАЛЬНЫХ НАРУШЕНИЙ ДЕФЕКАЦИИ

Функциональные аноректальные расстройства, включая хронический запор, недержание кала и синдром обструктивной дефекации (СОД), затрагивают от 10% до 25% населения развитых стран и оказывают значительное негативное влияние на качество жизни пациентов. Высокоразрешающая аноректальная манометрия (ВРАМ) произвела революцию в физиологическом исследовании аноректальной зоны, обеспечив одновременную регистрацию давления в высоком пространственном разрешении с отображением в виде цветных топографических карт — так называемых графиков Клауза. В отличие от традиционной манометрии с ограниченным числом сенсоров, ВРАМ предоставляет объективную, динамическую и воспроизводимую оценку тонуса сфинктера, ректоанальной координации и ректальной чувствительности. Настоящий обзор систематизирует данные о технических принципах и протоколе ВРАМ, нормативных значениях и их интерпретации, диагностической ценности метода при диссинергической дефекации, СОД и недержании кала, а также о месте ВРАМ в системе комплексной предоперационной диагностики в сравнении с дополнительными методами исследования. Полученные данные подтверждают, что ВРАМ является незаменимым предоперационным инструментом, обеспечивающим физиологическую стратификацию пациентов и оптимизацию хирургической тактики.
