RESULTS OF SURGICAL TREATMENT OF OLD DISCOSIONS OF THE RADIAL HEAD IN CHILDREN

Abstract

Target. improving the results of treatment of chronic dislocations of the radial head in children.

Material and methods. In the Samarkand branch of the RSSPMCTO, in the department of the consequences of childhood trauma, 83 patients were operated on with chronic dislocations of the radial head in 2019-2022. The main cause of the injury was traumatic (jumping or falling from a height) in 58 (69.9%) and unrelated causes in 25 (30.1%) patients. Patients underwent clinical and radiological methods of investigation. By types of operations performed: open reduction of the head of the radius, suturing of the stretched joint capsule with the annular ligament in the form of corrugation (41 patients in the control group) and open reduction of the head of the radius with capsuloplasty of the annular ligament (42 patients of the main group).

Results. Analysis of long-term results of treatment of patients showed that, in the 1st group of patients, excellent and good results were noted in 31 (75.6%) patients, satisfactory in 7 patients (17.1%), unsatisfactory results in 3 patients (7.3%) . In the 2nd group of patients, excellent and good results were observed in 39 patients (92.9%), satisfactory in 3 patients (7.1%), no unsatisfactory results were observed.

Conclusions. The fibrous-modified joint capsule allows the formation of a flap for the restoration (plasty) of the annular ligament in the surgical treatment of chronic dislocations of the radial head, and the restoration of the annular ligament from its own fibrous-modified joint capsule, in turn, keeps the heads of the radius in the reduced position. Analysis of the long-term results of treatment of patients showed an improvement in the results of surgical treatment from 75.6% to 92.9%.
