RESULTS OF THE APPLICATION OF ADVANCED OSTEOSYNTHESIS IN DIAPHYSEAL FRACTURES OF THE HUMERUS

Abstract

Fractures of the proximal femur are one of the most common injuries, especially in older patients. Most of them are low-energy due to osteoporosis. In case of fractures of the trochanteric region, due to the favorable blood supply, there is a good chance of consolidation. In this regard, treatment of fractures of the trochanteric region should be surgical in order to mobilize the patient as early as possible and thereby create conditions for consolidation of the fracture. In contrast to femoral neck fractures, osteosynthesis is mainly used as a surgical treatment for pertrochanteric fractures, which is also due to good prognosis for consolidation. However, surgical treatment is not possible for all patients. So if there are concomitant procedures, surgery may continue or may need to be postponed indefinitely. However, canceling the operation, in turn, can lead to an even worsening of the patient’s condition and the development of fatal consequences. In this regard, the lack of fixation of fractures causes severe pain and significantly worsens the quality of life of patients. To solve this problem, we have proposed a method for fixing a fracture of the proximal femur without general anesthesia by introducing percutaneous wires into the femoral neck without skin incisions and open reposition. Using the developed method, 4 elderly and long-lived patients with pertrochanteric fractures were treated, for whom extensive surgical intervention was contraindicated or there was a need to delay it.
