СОВЕРШЕНСТВОВАНИЕ МЕТОДОВ КОНСЕРВАТИВНОГО ЛЕЧЕНИЯ ПОВРЕЖДЕНИЙ СЕЛЕЗЕНКИ ПРИ ЗАКРЫТОЙ ТРАВМЕ БРЮШНОЙ ПОЛОСТИ

Цель исследования
Совершенствование комплексного подхода к ведению травм селезёнки при закрытых повреждениях живота.
Материалы и методы
В период с 2014 по 2024 год в Самаркандском филиале Российского Национального Центра Экстренной Медицинской Помощи (РНЦЭМП) было пролечено 29 пациентов с травмами селезёнки, полученными в результате множественных закрытых повреждений живота. Всем пострадавшим проводилась консервативная гемостатическая терапия.

При поступлении у всех больных с повреждением селезёнки были зафиксированы выраженные признаки внутрибрюшного кровотечения: абдоминальная боль, коллаптоидное состояние, положительные симптомы Керра, «ваньки-встаньки» и Куленкамфа.
По результатам ультразвукового исследования объём гемоперитонеума составил:В последнем случае для удаления крови была успешно применена видеолапароскопия.
Заключение
Анализ 29 пострадавших с закрытой травмой селезёнки показал, что на основе данных динамического наблюдения с помощью УЗИ, КТ и МРТ возможно безопасное и эффективное проведение консервативного лечения. Во всех случаях были достигнуты положительные клинические результаты без летальных исходов. Применение современных неинвазивных методов визуализации позволило своевременно установить диагноз и наблюдать пациентов в динамике, что обеспечило достаточный уровень диагностики без необходимости прибегать к видеолапароскопии или экстренному хирургическому вмешательству.
