ПЕРСПЕКТИВЫ ХИРУРГИИ С ПРОБУЖДЕНИЕМ В НЕДОМИНАНТНОМ ПРАВОМ ПОЛУШАРИИ ГОЛОВНОГО МОЗГА (обзор литературы)

Аннотация

Применение хирургии «в сознании» (awake surgery) в лечении диффузных глиом низкой степени злокачественности (ГНСЗ), особенно локализующихся в «функционально важных» областях мозга, стало рутинной практикой во многих нейрохирургических учреждениях мира и уже документируется как стандартная процедура и даже внесены в гайдлайны развитых стран мира. Хирургия «в сознании» или с «пробуждением» считается «золотым стандартом» при хирургической резекции глиом низкой степени злокачественности левого доминантного полушария. [1,2] Однако, интраоперационный мониторинг функций недоминантного полушария (ПндП) часто ограничивается только мониторингом двигательных путей и остальные функции игнорируются из-за догматического представления правого полушария как «малозначимого», а также с другой стороны, из-за малоизученности неврологических функций долей правого полушария, методов нейрокогнитивного тестирования сложных функций, таких как социокогнитивная функция, зрительно- пространственный гнозис, затрудняет работу специалистов по интраоперационному мониторированию.
